Pathway Home – North and North West

Pathway Home is a voluntary reunification program that works in collaboration with Child Safety Services to support children to be safely reunified with their family of origin; promoting the importance of felt safety, stability and wellbeing of vulnerable children, young people and their families.

The collaborative reunification process includes:

  • Working with the family to improve functioning, parenting, child behaviour’s and development
  • Improving the child’s sense of connection and attachment with the parent’s/family
  • Participation in decision making regarding reunification
  • Providing sustained and ongoing support during the reunification process and utilising any other appropriate services as required
